Lucrezia was the first great love of the Milanese prince and future Duke of Milan, Galeazzo Maria Sforza (1444–1476). She was roughly his age when their relationship began in 1459. As Lucrezia was the illegitimate daughter of Carlo de' Medici and of a lower social standing than the Milanese prince, marriage between the two was out of the question. Francesco Sforza and Bianca Maria Visconti - Galeazzo Maria Sforza’s parents - officially sanctioned the relationship to prevent their son, whom they could no longer control, from entering into a secret marriage with Lucrezia out of defiance. Lucrezia was married to a certain Giampietro Landriani, who served as a nominal husband so that the children born of the union between Lucrezia and Galeazzo Maria Sforza would not - unlike her - bear the heavy stigma of illegitimacy. Her children with Galeazzo Maria Sforza were:
- the son Alessandro (1460-1523), since 1474 Count of Galliate, since 1490 married to Countess Barbara Balbiani of Valchiavenna († c. 1520): one daughter: Camilla (1492-1554), who was married to Count Giulio Malvezzi († 1522/40) in 1506
- the son Carlo (1462-1483), since 1476 Count of Magenta; married to Bianca Simonetta († 1487), daughter of Count Agnolo Simonetta; two daughters: Angela and Ippolita
- the daughter Caterina (1463-1509)
- the daughter Chiara (1464/65-1531), her first husband was Pietro († 1485), Count dal Verme di Sanguinetto, Lord of Vigevano, they married in 1480; her second husband was Fregosino Campofregoso (1460-1512), Lord of Novi, they married in 1487 – Chiara had two sons from her second marriage: Paolo (1489-1545) and Ottaviano (1491/92-1525)
When the relationship between Lucrezia and Galeazzo Maria ended around 1465, Lucrezia lived with her nominal husband, to whom she bore three more children: Pietro, Stella and Bianca.
